Service Offerings and Packages
General Price List- Effective December 1, 2007
Please note: The categories as presented below are established by the Federal Trade Commission. Although these categories are intended to be helpful in their entirety, they can sometimes seem confusing. Please be sure to read on to the Lane-Moynihan Service Packages and LANE-MOYNIHAN Economy Assistance Plans, as these are presented concisely and are the affordable service offerings that is making Lane-Moynihan a preferred provider of funeral, cremation, and memorial services in Chicago.
The goods and services shown below are those we can provide to our clients.
You may choose only the items you desire, however any funeral arrangements you select will include a charge for our basic services and overhead. If legal or other requirements mean you must buy any items you did not specifically ask for, we will explain the reason in writing on the statement we provide describing the funeral goods and services you selected.
Basic Services of the Funeral Director and Staff
Our fee for the basic services of funeral director and staff includes, but is not limited to, staff to respond to initial request for service; arrangement conference with family and responsible parties; arrangement of funeral; preparation and filing of necessary authorizations and permits; recording vital statistics; preparation and placement of obituary notice; and coordination with those providing other portions of the funeral, e.g., cemetery, crematory, vault companies and others as required. Also included in this charge are overhead expenses relative to our facilities; such as professional licensing, legal and accounting fees, insurance, building and utility expenses, parking lot and grounds, maintenance, taxes, equipment, furnishings, inventory costs, record-keeping, and secretarial and administrative expenses. Also included are the special services of filing or assisting with insurance, union, social security, veterans or other burial claims or benefits that may be applicable, as well as notary public services.
This fee for our basic services and overhead will be added to the total cost of the funeral arrangements you select. (This fee is already included in our charge for direct cremations, immediate burials and forwarding or receiving remains) $1350.
Embalming
Except in certain special cases, embalming, is not required by law. Embalming may be necessary, however, if you select certain funeral arrangements, such as a funeral with viewing. If you do not want embalming, you usually have the right to choose an arrangement that does not require you to pay for it, such as direct cremation or immediate burial. $ 575.
Other Preparation of the Body
Dressing, and cosmetic restoration or General preparation, surface disinfection and dressing of unembalmed deceased. $ 175.
Refrigeration of unembalmed remains (it is the policy of our firm to require refrigeration of an unembalmed body after 24 hours.) $75 per day
Visitation
This fee includes the staff services required to monitor visitation periods (typically 2 or 3:00 -- 9:00 p.m.), assist at any ceremonies, direct guests, handle and place floral arrangements, memorial and Mass cards, coffee service and other various services, as well as the use of necessary funeral home facilities and equipment.
One day visitation $ 1250.
Two day visitation $ 1950.
Visitation at other facility $ 650.
Funeral/Memorial Services
This fee includes our services for coordinating and conducting ceremonies, staff to attend the service, supervision of committal at cemetery or other disposition service and the use of necessary funeral home facilities and equipment.
Service or Memorial Service at, or originating at, funeral home,
or at other facility on day after visitation $ 500.
Service or Memorial Service at, or originating at,
funeral home without visitation or with up to 2 hours visitation prior to service on same day $ 1050.
Memorial Service at other facility or use of equipment and staff
for church or graveside service $ 500.
Cemetery/Graveside Services Only
Our services include the funeral arrangement at the cemetery, supervision, staff, and equipment necessary to conduct services originating at the cemetery $ 250.
Automotive Equipment
Transfer of deceased (from home, medical facility or airport) $ 350.
Funeral Transportation to cemetery $ 450.
(Includes 40 miles. Long distance transportation: $2 per mile)
Forwarding of Remains to Another Funeral Home
Our charge for this service includes transfer of remains to funeral home, embalming, basic services of funeral director and staff, basic shipping container/air tray and local transportation to airport or other location. This charge does not include viewing, ceremony, merchandise or other transportation charges. $ 2175.
Receiving Remains from Another Funeral Home
Our charge for this service includes transfer of remains to funeral home, basic services of funeral director and staff, and transfer of remains to cemetery or crematory. This charge does not include viewing, ceremony, graveside service or merchandise. $ 1350.
Direct Cremation $ 1850.
The charge for a direct cremation includes:
“¢ Basic services of funeral director & staff, removal of remains from local place of death, transportation to crematory, procuring necessary authorizations;
“¢Cremation process including crematory fees.
“¢Minimal fiberboard cremation container and basic cremains container.
A complete selection of caskets, urns and upgraded cremation containers are available.
This charge does not include the use of facilities and staff for any visitation or ceremony.
Services to Anatomical Gift Association, $ 1500.
formerly known as Demonstrators Association
This charge includes:
“¢ Basic services of funeral director & staff, removal of remains from local place of death, transportation to Anatomical Gift Association, procuring necessary authorizations;
This charge does not include the use of facilities and staff for any visitation or ceremony.
Immediate Burial $ 1500.
The charge for immediate burial includes removal of remains, local transportation to the cemetery, necessary authorizations and basic services of staff. This charge does not include embalming or other preparation of the remains, use of facilities and staff for any visitation or ceremony.
Immediate burial with casket selected by or provided by purchaser
(The cost of a casket is not included in this fee.)
Lane-Moynihan Service Packages
1. One Evening of Visitation - Service Next Day $ 4,650.
Includes: Basic services, , transfer of deceased to funeral home ,embalming and other preparation, use of facilities for one evening (typically 2 or 3 to 9 p.m.) and service next day (either at funeral home, church and/or cemetery), transfer of deceased and hearse service.
2. One Evening of Visitation -- Private Transfer $ 4,150.
Includes: Basic services, embalming and other preparation, use of facilities for one day (typically 2 or 3 to 9 p.m.), with or without evening service, transfer of deceased to funeral home and private transfer to cemetery, crematory or airport.
3. Short Visitation Prior to Service Same Day $ 3,950.
Includes: Basic services, embalming and other preparation, use of facilities for one or two hour visitation, followed by chapels service, transfer of deceased to funeral home and funeral transportation to cemetery or crematory.
4. Direct Cremation $ 1,850.
The charge for a direct cremation includes:

5. Direct Cremation with Memorial Service $ 2,750.
Includes: Basic services, other preparation of body, use of facilities for evening memorial service (can include typical visitation hours), transfer of deceased to crematory
6. Direct Cremation with Private Family Identification $ 2,200.
Includes: Basic services, other preparation of body, private family viewing, transfer of deceased to funeral home and crematory.
7. Graveside Services $ 2,550.
Includes: Basic services, embalming and other preparation, transfer of deceased to funeral home and funeral transportation to cemetery.
LANE-MOYNIHAN Economy Assistance Plans
These three discounted plans are available to
those who demonstrate financial hardship.
These packages must be taken in their entirety;
there can be no reduction in price for any items
in the package that are not wanted or utilized.
The terms for these packages are exactly as described.THERE CAN BE NO EXCEPTIONS OR SUBSTITUTIONS.
We ask that the discounted package be paid for at the time of service to reduce our administration expenses.
Plan A: Three Hour visitation/Service next day/Hearse to cemetery $ 4,950.
This discounted package includes the basic services of the funeral director and staff; embalming and other preparation of the body; visitation for three hours funeral next day originating at funeral home or church, transfer of deceased, hearse to cemetery; and a casket.
Plan B: Three Hour visitation/Service same day/Hearse to cemetery $ 4,450.
This discounted package includes the basic services of the funeral director and staff; embalming and other preparation of the body; visitation for three hours funeral same day originating at funeral home or church, transfer of deceased, hearse to cemetery; and a casket.
Plan C: Three Hour visitation/Service same day/Private transfer
$ 4,150.
This discounted package includes the basic services of the funeral director and staff; embalming and other preparation of the body; visitation for three hours funeral same day originating at funeral home or church, private transfer of deceased to cemetery or crematory; and a casket.

Warranties:
Lane-Moynihan Funeral & Cremation, LLC makes no representations or warranties about the protective value of certain caskets or outer burial containers other than those made by the manufacturer. The only warranties, expressed or implied, granted in connection with goods sold with this funeral service are the written warranties, if any, extended by the manufacturers thereof.
No other warranties and, specifically, no warranties of merchantability and fitness
for a particular purpose are extended by the seller.
